Through CAJA — Your Medications at No Cost
As a CAJA enrollee, you have access to the CAJA medication formulary — a comprehensive list of approved medications dispensed at CAJA pharmacies at no additional cost to you. The formulary covers most common medications for chronic conditions including hypertension, diabetes, cholesterol, thyroid disorders, depression, anxiety, and many others.
To receive medications through CAJA, you need a prescription from your assigned EBAIS physician. Once established in the system, most patients receive ongoing refills through their regular CAJA clinic visits. The medications dispensed are typically generic equivalents of U.S. brand-name drugs — manufactured to international standards and therapeutically equivalent.
The CAJA formulary does not include every medication available in the U.S. Specialty medications, newer brand-name drugs, and some less common medications may not be available through CAJA. For these, private pharmacies are the solution.

Puerto Viejo and the Limón region have several well-stocked private farmacias. For specialty medications not available locally, a trip to San José or an order through a San José pharmacy with delivery service is the standard solution.
Bring a three-month supply of all critical medications when you first move to Costa Rica. This gives you time to establish care with a local CAJA physician, build the relationship needed for ongoing prescriptions, and locate any specialty medications locally without any gap in treatment.
Controlled Substances and Special Considerations
Controlled substances — including opioid pain medications, benzodiazepines, ADHD medications, and certain sleep aids — are heavily regulated in Costa Rica and require a Costa Rican prescription from a licensed physician.
