Medications in Costa Rica — Better Than Most Retirees Expect

One of the most common worries for American seniors planning a move to Costa Rica is medication access: Will my prescriptions be available? Will I be able to afford them? What if I run out? The reassuring reality is that Costa Rica has a well-developed pharmaceutical infrastructure, and the vast majority of medications used by American seniors are available — either through the CAJA system free of charge, or through private pharmacies at costs that are dramatically lower than U.S. retail prices.

Through CAJA — Your Medications at No Cost

As a CAJA enrollee, you have access to the CAJA medication formulary — a comprehensive list of approved medications dispensed at CAJA pharmacies at no additional cost to you. The formulary covers most common medications for chronic conditions including hypertension, diabetes, cholesterol, thyroid disorders, depression, anxiety, and many others.

To receive medications through CAJA, you need a prescription from your assigned EBAIS physician. Once established in the system, most patients receive ongoing refills through their regular CAJA clinic visits. The medications dispensed are typically generic equivalents of U.S. brand-name drugs — manufactured to international standards and therapeutically equivalent.

Important Caveat

The CAJA formulary does not include every medication available in the U.S. Specialty medications, newer brand-name drugs, and some less common medications may not be available through CAJA. For these, private pharmacies are the solution.

Through Private Pharmacies — What's Available and What It Costs

Costa Rica's private farmacias carry an extensive range of medications, including many brand-name drugs and a wide variety of generics. Prices are regulated by the government and are dramatically lower than U.S. retail prices for most medications. Common examples:

  • Metformin (diabetes): ~$5–$12/month private vs. $15–$30 U.S. retail
  • Lisinopril (blood pressure): ~$4–$8/month private
  • Atorvastatin (cholesterol): ~$8–$15/month private
  • Omeprazole (acid reflux): ~$5–$10/month private
  • Levothyroxine (thyroid): ~$6–$12/month private

Puerto Viejo and the Limón region have several well-stocked private farmacias. For specialty medications not available locally, a trip to San José or an order through a San José pharmacy with delivery service is the standard solution.

What to Bring When You First Arrive

Bring a three-month supply of all critical medications when you first move to Costa Rica. This gives you time to establish care with a local CAJA physician, build the relationship needed for ongoing prescriptions, and locate any specialty medications locally without any gap in treatment.

Controlled Substances and Special Considerations

Controlled substances — including opioid pain medications, benzodiazepines, ADHD medications, and certain sleep aids — are heavily regulated in Costa Rica and require a Costa Rican prescription from a licensed physician. You cannot bring large quantities from the U.S. Bring only what you need for the transition period and arrange for a local prescription as soon as possible. At Magnolia Reserve, our on-site physician can assist with prescription transitions for new residents.

Some medications that require a prescription in the U.S. are available over the counter in Costa Rican pharmacies — including many antibiotics and common medications for blood pressure and pain. This can actually be convenient for minor acute illnesses, though it is always preferable to establish a relationship with a local physician for ongoing care.
